If you see something, say something! Use this form to report incidents of concerning
behaviors among student/s (CARE Team) or student misconduct/criminal activity on-campus,
online, or during any NMJC-sponsored events or programs. The NMJC Code of Conduct
can be found in the
NMJC Student Handbook. Some examples of concerning behaviors include threats, aggression, emotional distress,
social withdrawal, and sudden mood changes. Some examples of Code of Conduct violations
include possession or use of alcohol or other drugs, stalking, physical violence or
assault, harassment, disruption of the educational process, and failure to comply
with a college official’s instructions. Please do not use this form to report emergencies.

such as damage to college property caused by weather, facilities problems, a minor
car accident involving a campus visitor, any non-student criminal activity, an incident
in which no person is involved but which may affect the campus climate and/or safety,
or incidents involving NMJC faculty or staff. Please do not use this form to report

Use this form to report any instances of sexual assault, stalking, domestic or dating
violence, or discrimination/harassment based on gender or sex against ANY person (you
are not required to provide your name). This form should NOT be used to report immediate

Hazing Reporting Form
Use this form to report any instances of hazing which includes any act committed as
part of a person’s recruitment, initiation into, pledging, admission into, or affiliation
with a student organization, athletic team, or living group, or any pastime that is
likely to cause, bodily danger or physical harm, or serious psychological or emotional
harm, to any student. This includes causing, directing, coercing, or forcing a person
to consume any food, liquid, alcohol, drug, or other substance that subjects the person
to risk of such harm, regardless of the person’s willingness to participate. This
