NMJC Foundation - Agency Accounts
The Foundation needs a completed Memo to Change Account for each agency account. This memo needs to be completed by the Sponsor(s)/Authorized Person(s). This memo needs to be completed and returned to the Foundation every time there is a change in who is authorized to withdraw funds from the account.
A Memo to Deposit form needs to be completed any time funds are deposited in the agency account.
To withdraw funds from your agency account, please complete the Memo to Withdraw/Pay from Account. If you are charging purchases to your NMJC Credit Card, please send an email to firstname.lastname@example.org of the anticipated charge before making the purchase. Once the purchase is complete and the credit card statement needs to be paid, please complete the Memo to Withdraw/Pay from Account. The NMJC Foundation will then pay the NMJC Business Office the portion of your credit card bill that is to come from your agency account.
